Duplicate high school diplomas are not provided. Copies of school transcripts or records for students that graduated or withdrew from a Broward County Public School more than five years ago may be obtained by submitting a completed Transcript Request Form available online at (http://www.broward.k12.fl.us/etsweb/records_retention_transcript_req.htm) or by calling 754-321-3150.

The Broward County School District is a GED testing facility. All requests for verification of GED test scores and official GED transcripts are handled by the Florida Department of Education, GED Testing Office through their Web site (http://www.firn.edu/doe/workforce/ged_dipl.htm) or call them toll free at 877-352-4331.

Yes you may. There is a $3.00 fee for obtaining immunization records, and the acceptable forms of payment are cash, credit card ( MasterCard or Visa), and checks. (Make checks payable to UNO Student Health Services along with your SSN on the check.) Out-of-state students who wish to receive their immunization records from UNO must send in a request letter along with the fee of $3.00.

You may request your medical records by printing and filling out an authorization allowing us to release your private health information. You must then send us the form by mail to: The form must be filled out completely in order to process your request. Patients and legal offices (attorneys) obtaining medical records will be billed $0.75 per page to cover the cost of copying fees. Life Insurance companies and their representatives will be billed a flat rate of $175.00.

Submit a diploma re-order form to the Office of the Registrar with a copy of the $20 receipt from the Office of Payments and Collections.

To obtain a copy of your Secondary School Diploma or Diploma of Vocational Studies, simply send in a written request to the Direction de la sanction des études, indicating your name in full, the address where you would like the diploma sent, your permanent code or date of birth, the name of the school you attended and the municipality in which it is located, the year the diploma was issued, and, if applicable, the terminal year (e.g. grade 11). Please make sure you sign this request.

A student can obtain a copy of a lost Diploma or Certificate by contacting IATA Training. An administrative fee of USD 50 per Diploma or Certificate will be charged to prepare and forward the Diploma or Certificate to the student.

You must sign a "Release of Records" Form, which can be obtained on-line or at any PMG office. A contracted copy service usually performs this service and a small fee may be charged.

Each school district maintains its own records so you will have to contact your local school to inquire about high school transcripts. You will find a complete list of Alaska schools and school districts, their addresses, contact names, telephone numbers, and e-mail and website addresses (for those school districts that have them) on the Rolodex page of this website.

Check with your family to see if they have a copy of your immunization records available. A copy of your records may also be on file with your previous place of education (high school, undergraduate university). Records may also be found at your doctor's office. Please be aware that these records may not be up to date. If you are sure that you have been immunized against the measles, mumps, and rubella, and/or the chicken pox, you have the option of getting your titers drawn.

Health records are confidential. You can request a copy of your records in writing. The letter must state that you give the College of Staten Island permission to release your immunization and/or medical records. Then give the name, address, or fax number of the agency you want to receive your records. The letter must also be signed and include your student identification number. The letter can be faxed to us or you can go to the Health Center and fill out a Medical Record Request Form.

Patients may request a copy of their medical records for their personal use or for delivery to a physician outside of Urology San Antonio by submitting the Medical Records Request form found on the Medical Records page of our website.

Talk with your dentist about getting a copy of your dental records. Dentists covered by the HIPAA privacy rule are required to provide patients with a copy of their records and state law may also apply.

To request a copy of your graduate transcripts, students must contact the Transcript Request Line at (937) 775-5599 or contact the Registrar at (937) 775-5588.
