this time, the program will only be open to eligible faculty and staff. The current plan identifies a phased approach with the possibility of opening participation to retirees in approximately 2-3 years. ^ TOP You will only be contacted by a UI Wellness staff member if you have given your permission to do so. Within the written report that you receive, resources will be identified that you can contact directly for assistance in the event that you do not give permission.

All state and university employees are eligible to participate in this benefit program. Also eligible are the employees of the approximately 700 local government and school district employers that have adopted the WDC.

To participate in our program, your organization must be a registered nonprofit. This can include schools, charities, hospitals and clinics, volunteer services, political organizations, fraternal organizations, professional associations, religious organizations, governmental agencies, etc. Each organization submitted will go through an approval process to confirm their legitimacy. Upon approval they will be notified with a welcome e-mail.

EBSA will consider an application if neither the plan nor the applicant is "Under Investigation" (as defined in the Program) and if the application contains no evidence of potential criminal violations as determined by EBSA. We have made the Program available to more applicants by revising the definition of who is "Under Investigation."

Chapter 61 retirees who are also eligible for longevity retirement (20 years or more for retirement purposes) will be eligible for CRDP and CRSC. However, their entitlement will be calculated as if they had received a non-disability retirement. They will not be reimbursed to the level of their service-awarded disability retirement. Chapter 61 retirees with fewer than 20 years of service are not eligible for either program at this time.

A qualified family must include a minimum of four clinically diagnosed male prostate cancer cases, three of whom must be available for testing. A minimum of eight family members should be tested including unaffected members (sisters, mother or father of affected, and aunts or uncles on affected side). The following families will be most informative: (1) families with prostate cancer on one side of the family, i.e.

Women at increased risk for developing breast cancer, who have gone through menopause and are at least 35 years old, can participate in STAR. All women must have an increased risk of breast cancer equivalent to or greater than that of an average 60- to 64-year-old woman. At that age, about 17 of every 1,000 women are expected to develop breast cancer within five years.

Regular, full-time employees regularly scheduled to work at least 30 hours a week are eligible to enroll in an FSA. Limited-term employees are not eligible.
