Once we receive complete information from the input forms (or acceptable substitutes) on the restaurant, it takes 3-5 business days to process your Restaurant Valuation Report. The RVP will be shipped out according to your shipping instructions - when you put down your deposit. Upon receipt of your valuation report - the remaining balance is due.

It is recommended that you first seek medical attention from your physician. They may ask you to submit a stool sample to test for foodborne pathogens. This testing, while not pleasant, provides significant information in our investigation. If your physician suspects that you have a foodborne illness and the establishment is licensed by FVHD, you can contact us at 676-1953 to file a complaint.

The inspection reports for every food facility in Orange County are available to the public for examination. To obtain access to the reports, please contact the custodian of records at (714) 834-2147.

The Environmental Health Division conducts inspection on all restaurants within the city. If you want to report an unclean restaurant, or food illness, please call (804) 646-3120.

Equines, llamas, and alpacas make up an important component of the Maine CDC Arboviral Surveillance program. Owners and veterinarians are urged to report cases of illness in these species so that the Maine CDC can determine if arboviral testing is appropriate.

If a student will not be in school, please call the office by 9:00 a.m. so that the appropriate teacher may be notified. The school must be notified immediately if your child has a communicable disease or condition such as chicken pox, measles, or lice. Please inform your child's teacher as soon as your child is diagnosed to permit the school to alert the other families in he school. The school nurse is located on the fifth floor.

One of the major advantages of the LAP-BAND® System is that it can be adjusted. If your illness requires you to eat more, the band can be loosened. This can be done by removing saline from it. When you have recovered from your illness and want to lose weight again, the band can be tightened. This can be done by increasing the amount of saline. If the band cannot be loosened enough, it may have to be removed.

We do not have a restaurant on the premises however we do serve room service breakfasts and there are several good quality restaurants and cafes within a short walking distance (500m).

No, you do not need to have a computer at the restaurant. You do need access to a computer to configure and manage your site -- this can be done with any computer. In this case, you would choose to receive orders by fax.
