For births in one of the five (5) boroughs of New York City (Manhattan, Kings, Queens, the Bronx, and Staten Island), please visit the New York City Department of Health and Mental Hygiene web site. Please note that the borough of Kings is sometimes referred to as Brooklyn and the borough of Staten Island is sometimes referred to as Richmond.

You may request your medical records by printing and filling out an authorization allowing us to release your private health information. You must then send us the form by mail to: The form must be filled out completely in order to process your request. Patients and legal offices (attorneys) obtaining medical records will be billed $0.75 per page to cover the cost of copying fees. Life Insurance companies and their representatives will be billed a flat rate of $175.00.

A birth certificate may be obtained from the local registrar of the city you were born in or from the New York State Department of Health. You may apply online at http://www.health.state.ny.us/nysdoh/vr/forms/birth.htm

You must either come in person to our office with picture identification, or submit a written request. The written request must include full name at birth, date of birth and both parents' names. You MUST SIGN your request and give an address where we are to mail the certified copy. The fee for the certified copy must accompany your written request. The request is filled the day we receive it and it goes out in the next business day's mail.

Only your current spouse, brother/sister, child/step-child or grandchild, parent/step-parent or grandparent, authorized agent, attorney or legal representative is authorized to obtain a certified copy of your birth certificate. The Application for a North Carolina Birth Certificate form completed.

Birth or death certificates or copies of the same are available from the Clerk and Recorder's Office at the Courthouse, 155 West Granite, Butte, Montana, Room 201. Certified copies of birth certificates are $5.00; death certificates are $3.00.

Patients may request a copy of their medical records for their personal use or for delivery to a physician outside of Urology San Antonio by submitting the Medical Records Request form found on the Medical Records page of our website.

The Broward County School District is a GED testing facility. All requests for verification of GED test scores and official GED transcripts are handled by the Florida Department of Education, GED Testing Office through their Web site (http://www.firn.edu/doe/workforce/ged_dipl.htm) or call them toll free at 877-352-4331.

Our office keeps records only for births occurring in Ashe County. Requests are honored by mail or in person.
