What is CRT-D?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
CRT-D is a type of biventricular pacing (helps both ventricles contract at the same time). Its indications differentiate CRT-D from other forms of pacing. In practical terms, patients with bradyarrhythmia receive a conventional pacemaker, patients with heart failure alone receive CRT-P (pacing alone) and patients with tachyarrhythmia alone receive a conventional implantable cardioverter defibrillator (ICD).

What is a CRT-D device?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
A CRT-D device is an ICD enabled with Cardiac Resynchronization Therapy (CRT). This device is implanted into a patient as part of a system that includes leads. Leads are specialized, thin, insulated wires that attach to the CRT-D device and deliver the therapy to the patient’s heart. CRT-D devices may also be referred to in physician operative notes by one of the following terms:

Who makes CRT-D devices?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
Currently, CRT-D devices approved for use by the FDA are manufactured by only three companies: Medtronic, Guidant and St. Jude Medical.

What are the coding challenges in CRT-D?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
The fact that both an arrhythmia and a heart failure diagnosis would be present for CRT-D may lead to uncertainty in selection of the principal diagnosis. The circumstances of admission must be clearly documented to indicate the role both diagnoses play.

How much is the CRT-D add-on payment worth?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
The add-on payment is calculated on a case-by-case basis. The payment amount can range from $0 up to a maximum of $16,262.50. The actual payment for an individual case is roughly determined by the expected financial “loss” on that case. If the DRG payment adequately covers the cost to treat the patient, there is no payment.

Can any company's CRT-D devices qualify for this payment?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
Yes. When Medtronic decided to apply for this payment on behalf of our hospital customers, we wanted to make sure that hospitals could maximize their total benefit by ensuring that any CRT-D case could qualify. Accordingly, products from Medtronic, Guidant, and St. Jude Medical all qualify as long as they are CRT-D devices. Ask your Medtronic representative for a reference card that outlines all of the CRT-D devices currently approved for use in the US.

What are the requirements for my hospital to qualify for a CRT-D add-on payment?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
The CRT-D new technology add-on payments are provided on a case-by-case basis. In order to qualify, the case must involve the following:

Why does the principal diagnosis matter for CRT-D implants?

Coding and Reimbursement Update for Cardiac Resynchronizatio...
In 2003, defibrillator DRGs and their associated description changed. When a patient has a principal diagnosis of heart failure (HF), acute myocardial infarction (AMI), or Shock, an EP study and CRT-D implant will result in assignment to DRG 535. However, if HF, AMI or Shock is not indicated as a principal diagnosis, it results in assignment to DRG 536, a lower paying DRG.

What is cardiac resynchronization therapy (CRT)?

Boston Scientific ::
Cardiac resynchronization therapy (CRT) is one treatment option for some heart failure patients. CRT uses electrical impulses to help your heart pump more effectively. In a healthy heart, the chambers contract and relax in a coordinated way, or in synchrony. Heart failure can cause problems with the timing or coordination of your heart's contractions — especially in the ventricles (lower chambers). In other words, it can cause dyssynchrony.

What About Vitamin D?

FAQ For Vegetrial World
Preformed vitamin D is not needed by the body, it can be synthesized by exposure to sunshine of dehydrocholesterol present in the skin. Vitamin D created this way lasts in the body for many months such that it is possible to "top-up" one's vitamin D levels over the summer for the coming winter. Excess consumption of pre- formed Vitamin D can be dangerous. The Vitamin D in cow's milk is artificially added.

I-D. What is cryopreservation ?

CRYONICS − FREQUENTLY ASKED QUESTIONS (FAQ)
Cryopreservation is the use of antifreeze (cryoprotectant) solutions and of cooling to very low temperature on humans, animals, organs or tissues for long-term storage − typically liquid nitrogen temperature (-196°C). If freezing can be avoided − as with vitrification − cryopreservation is not freezing.

What are the functions of the CRT?

CRT-II - Claims Resolution Tribunal - Deposited Assets
The CRT .s task is to process individual claims to Swiss bank accounts of claimants who seek the return of assets deposited in Swiss banks. In this regard, the CRT determines whether each particular claimant has plausibly demonstrated that an account was opened by a Victim or Target of Nazi Persecution and whether the claimant is entitled to the proceeds of that account.

Who should be the trustee of the CRT?

Welcome to WISEMAN BIGGS BRAY PLLC
You can be your own trustee. But you must be sure the trust is administered properly, otherwise, you could lose the tax advantages and/or be penalized. Most people who name themselves as trustee have the paperwork handled by a qualified "third party administrator." However, because of the experience required with investments, accounting and government reporting, some people select a corporate trustee (a bank or trust company that specializes in managing trust assets) as trustee.
